









File of Life
When responding to a medical emergency in the home the Emergency Medical Technician
needs information such as blood type, medication being taken and allergies. It is possible
that the victim is alone and cannot respond to questions or, if other people are in the home,
they cannot provide the information. The File of Life, a 4"x5" red vinyl folder with a paper
insert and a magnet, contains the basic information needed by the responder. The resident
completes the form and the folder is placed on the refrigerator. Emergengy responders in
Fairview are trained to go immediately to the refuigerator in a home that has a small red
decal, included in the package, containing the words "File of Life" on a front door window
or sidelight.
Through the generosity of AMR, the medical emergency responder serving Heritage Ranch
under a contract with the Town of Fairview, sets of this document were provided to the
Heritage Ranch Community Emergency Response Team. You may pick up your free "File of
Life" at the Front Desk in the Clubhouse. Request extra forms for each member of your
household.
